Peer Support Training
Transitioning home from an inpatient hospital stay can be difficult, particularly for people living with complex and comorbid conditions. Peer support, provided by trained community members who share lived experience with their clients, can be a helpful complement to outpatient clinical care in order to help people maintain health progress that they achieved in hospital post-discharge.
We have conducted research piloting peer support to ease hospital discharge for people living with HIV, and have developed a training manual for peer support personnel in complex care.
